Abstract:

Background: It has not been yet developed a ost suitable method to treat right ventricular failure due to pulmonar artery hypertention resulted from ventricular arrhythia. Though some case reports about patients ready to heart transplant using left assist device or biventral assist device as bridge have been published in Journals, but nocase concerning application of centrifugal pump for treatment of right ventricular failure has been reported. Case report: The patient was a 51- year-old woman from rural region of Kurdistan with sever mitral stenosis and left arterial clot and sever pulmonary hypertension who underwent mitral valve replacement and removing of clot. The patient developed right ventricular failure due to sever pulmonary hypertension (110 mmHg) when was waned from cardiopulmonary bypass therefore CPB was resumed to be followed by intra aortic bollon pumping howevwr complete response was not be obtained due to repeated ventricular tachycardia and fibrillation thus right ventricular support was performed using a centrifugal pump and the patient could be weaned from CPB, 2 day after surgery. The right ventricular support was discontinued and IABP was removed 7 day after surgery without marked changes in homodyamics. Conclusion: Although right ventricular failure due to sever pulmonary hyperlension is an ominous complication after open heart surgery but simultanrous use of ballon pumping and bio pumping for right ventricular failure in our patient caused her recovery.

Severe pulmonary hypertension and right ventricular failure

Perioperative management of severe pulmonary hypertension (PH) is challenging. Anaesthesiologists come across perioperative management of such cases during incidental surgeries, surgery for various congenital heart diseases and valvular heart diseases and for caesarean section or painless labour in pregnant patient with Eisenmenger syndrome. Knowledge of pathophysiology of PH and novel drugs ac...

Evaluation of Advanced Echocardiographic Parameters of Right Ventricular in Patients with Pulmonary Hypertension

Background and Objective: Pulmonary hypertension is an increase in blood pressure of the pulmonary artery which leads to right ventricular dysfunction. Therefore, the present study aimed to compare echocardiographic parameters in the study of right ventricular failure in patients with pulmonary hypertension. Materials and Methods: In the present descriptive cross-sectional study, data were col...

Pulmonary hypertension due to a pulmonary artery leiomyosarcoma: A case report

Background. Primary pulmonary artery sarcomas are very rare and their histologic type, called leiomyosarcoma, is even rarer. Case Report. A 64-year-old woman presented with progressive weakness, fatigue, malaise, and dyspnea, and a marked elevation of pulmonary artery pressure was admitted. She was initially diagnosed with chronic pulmonary thromboembolism and chest computed tomography (CT) sca...

Combined atrial septostomy and oral sildenafil for severe right ventricular failure due to primary pulmonary hypertension.

Management of primary pulmonary hypertension is usually difficult because the disease is uncommon and the aetiology of the disease is not well understood. The disease is potentially lethal because it can lead to failure of the right ventricle, low cardiac output, and ensuing multiple organ failure. We report the successful treatment of a case of low-output syndrome due to primary pulmonary hype...
